INTERNSHIP OPTIONS

Note: Attachments should be downloaded from the hyperlinks. The link to the student and company survey is available on the Teams platform in the “Praktyki” file (see: 0800-Praktyki team) or can be shared by internship supervisor.

Internship documents should contain: 
•    work diary
•    agreements (2 copies)
•    referral
•    accident insurance declaration 
•    learning outcomes attachment 
•    student questionnaire
•    company questionnaire

Below there are options used in justified situations:
2.    Internship from the employment bureau:
•    internship referral to an organisation (bring original for inspection + copy), or
•    agreement with the organisation where the internship takes place (bring original for inspection + copy)
•    mentor`s internship report or confirmation of the internship
•    acceptance of other forms of student internship programme
•    internship report (regarding activities and tasks within internship)
•    learning outcomes report  
•    student questionnaire
•    company questionnaire

3.    Self-employment
•    printout from the National Company Register 
•    tax identification number certificate (Polish NIP or REGON)
•    acceptance of other forms of student internship programme
•    internship report (regarding activities and tasks within internship)
•    learning outcomes report  

4.    Employment contract or civil contract 
•    agreement confirming employment (bring original for inspection + copy),
•    job description (if employment contract) or a list of tasks (if civil contract) 
•    acceptance of other forms of student internship programme
•    internship report (regarding activities and tasks within internship)
•    learning outcomes report  
•    student questionnaire
•    company questionnaire

5.    Own farm, horticulture, orchard, breeding, etc.
•    for farmers (owners): notary deeds (min. 2 hectares), or
•    alternatively: cultivation licence (bring original for inspection + copy), confirmation from the local government, certificates, completed or ongoing projects, other
•    acceptance of other forms of student internship programme
•    internship report (regarding activities and tasks within internship)
•    learning outcomes report  

6.    Active participation in organizational/scientific activities at the University of Łódź (including scientific clubs)
•    work diary
•    referral (used only if internship work is in favour of the University of Łódź)
•    accident insurance declaration 
•    acceptance of other forms of student internship programme
•    internship report (regarding activities and tasks within internship)
•    learning outcomes report

7.    Participation in a science camp
•    work diary

8.    Student participating in student exchange programs (e.g. Erasmus, Erasmus+, MOST, Campus Europae) or volunteering

•    learning outcomes report  
•    student questionnaire

9.    Paid internship in a university competition or organised by other institutions

If you plan to complete the internship programme started at an earlier date / or the internship programme is completed before the formal period, then you need to fill in Attachment 6.

In the case when internship cannot be done in the required period of time, it is necessary to submit to the Dean's Office application for postponing the session together with the opinion of appropriate internship supervisor.

Work diary, referrals and agreements are issued by the appropriate internship supervisor.
Internship offers for students are available in the Teams application in the 0800-Praktyki team
Note: internship offers from the Professional Career Office (Biuro Karier Zawodowych) at the Faculty of Management cannot be included in the study programme.
